This album is a concept album based on the horror novel of the same title, "To Kill to Live to Kill" (319 pages), written by vocalist Lars F. Larsen. The story is told in three parts, with each individual story focusing on a young man with a cursed sword, a hitman hired for money, and a deranged psychopath, and the fate of all three is revealed at the end.
